Eligibility for Minnesota Vaccine for Children Program
 
 
Children 18 years of age & younger 

  • No health insurance  
  • Have health insurance, but the plan does not cover the cost of vaccines.  Examples:  Covers only selected vaccines; no vaccine coverage; or have reached a cap in coverage.
  • Covered by MN Health Care Program (MHCP): children only

     Please present your program card at the time of immunizations.
 
 
Eligibility for UUAV Program (uninsured/underinsured adults)
 
 Adults 19 years and older 

  • No health insurance
  • Have health insurance, but the plan does not cover the cost of vaccines.  Examples:  Covers only selected vaccines; no vaccine coverage; or have reached a cap in coverage.
  • Adults on MN Health Care Program- see your physician for vaccine.
  • Vaccine availability for adults is subject to change depending on program funding. 

Things to remember:

Administration fee of $10 per immunization.  No one is refused for inability to pay this fee.  

Please bring immunization records to your appointment.

Immunization services given at public clinics should not be substituted for a child’s usual well-child visit with his or her primary care provider.

All minors under the age of 18 years must be accompanied by their parent or a legal guardian.
 
 

  
Recommended Immunization Schedules
Some people call them immunizations. Others call them vaccinations or shots. Whatever you call them, immunizations are one of the best weapons we have against a number of serious diseases. There would be serious consequences if the world would stop immunizing.

Flu Shot Information
Looking for a flu shot? Contact Pauline Amundson with Disease Prevention and Control at Chisago County Public Health by calling 651-213-5225 for an appointment.

Flu vaccinations are offered throughout the flu season - starting middle to late September.

Mantoux Testing
Mantoux (Tuberculosis screening skin test) testing is available to the public at a cost of $15. Situations where a Mantoux may be needed are for employment in a health care setting; post-secondary school enrollment in health care programs or if you are identified as a contact in a TB investigation. The test is placed and then read 48-72 hours later. This test if offered on weekdays except Thursdays.
