Assessment & Planning

Chisago County Public Health's work is guided by several department plans and data.

Community Health Assessment (CHA)

The CHA is the result of a collaborative community effort. It is a core function of public health, as well as a state mandated responsibility to conduct a community health assessment at least every five years. The report provides the general public and policy leaders with information on the health of our county's population.
